What to expect in pelvic therapy for endometriosis?

This blog is in honor of endometriosis awareness month (March). I thought I would highlight that pelvic therapists are a great ally if you are on a journey to heal endometriosis. "Did she say heal?" You heard that right. Stay tuned.


Here is what to expect in pelvic therapy for endometriosis:


  • Expect to have multiple session with the pelvic floor therapist to achieve results.


  • During an initial evaluation, you will discuss your symptoms, medical history, and how endo affects your day-to-day. The pelvic floor therapist will ask questions to help them understand your experience and what you hope to accomplish through therapy. Therapy is not one-size-fits-all. 


  • Expect for your pelvic therapist to complete a pelvic floor muscle assessment. This may happen the first visit and day of your initial evaluation or in a follow-up session. This is an internal exam similar to a pelvic exam. Your therapist will assess the strength of your pelvic floor muscles, m will assess the tone of your muscles, will help find tender muscles/trigger points, will check the coordination of these muscles, and then they will develop a program specific to your needs. 


  • Ongoing therapy sessions, depending on your needs may include the following:

    • Manual therapy to help manage scar tissue and abdominal bloating or manual therapy in the form of providing joint mobilization or soft tissue mobilization

    • Internal manual/physical therapy

    • Stretches

    • Various Soft tissue mobilization techniques (such as trigger point, skin rolling, manual stretching, deep tissue massage, using tools such as vaginal dilators/pelvic wands

    • strong focus on breathwork 

    • Yoga or Pilates 

    • PF/Inner Core Strengthening and coordination exercises


  • Recommendations may be made for a home exercise program (focusing on strength and flexibility) - practicing techniques and PF exercises outside of therapy sessions will help you progress 


  • The Length of the pelvic therapy program varies depending on the severity of your condition and symptoms 


One retrospective study found that 63 percent of women living with endometriosis experienced less pain after six sessions with a pelvic floor therapist. Another small study found that women living with endometriosis experienced less pain and improved posture after completing an 8-week pelvic exercise regimen.


  • Your pelvic floor therapist will collaborate with you to determine the best course of therapy for you.


  • A quantum pelvic floor therapist will also look at your case through a quantum lens and also incorporate non-traditional quantum modalities that help with better outcomes and overcoming plateaus.
